Keeps playing through doc social Morning Brief

3 stories this morning, for surgeons and the people who support them.

  • icotec's CMORE CT carbon/PEEK cervicothoracic system gets a Medicare new technology add-on payment, up to $60,905 per case, for fusion use only — icotec announced Oct 6, 2026 that CMS approved an NTAP for the CMORE CT System (BlackArmor Carbon/PEEK posterior cervico-thoracic fixation), effective Oct 1, 2026; the release does not give a dollar amount. Primary-source verification: FY 2027 IPPS final rule (CMS-1849-F), Federal Register 2026-15833, published Aug 4, 2026 (https://www.federalregister.gov/documents/2026/08/04/2026-15833), section on CMORE CT System: finalized under the alternative pathway for transformative devices; cost per case $93,700; maximum NTAP $60,905 (65% of average cost); limited to the Breakthrough-designated indication, adjunct to fusion of C1-C7 and T1-T3; use without fusion in advanced cervical tumor patients with short life expectancy is not covered by the NTAP; FDA marketing authorization as a Breakthrough Device Nov 12, 2025; newness period begins Dec 8, 2025 (commercial availability); unique ICD-10-PCS codes from FY 2027. The release's claim that CMORE CT is the only FDA-approved posterior cervicothoracic system for spinal infection is the company's claim and is not in the rule; the script does not state it as fact. Earlier NTAP coverage in the 2026-10-02 draft was PearlMatrix P-15 and INFUSE ($3,380 and $4,396.60 caps), no icotec/CMORE/carbon coverage in any prior draft (grep). Source
  • Stryker names Spencer Stiles CEO effective January 1, 2027; Kevin Lobo becomes Executive Chair — Stryker Form 8-K (SEC accession 0001193125-26-414956), filed Oct 6, 2026; the Transition Agreement with Kevin A. Lobo is dated Oct 5, 2026. Lobo remains CEO through Dec 31, 2026, then serves as Executive Chair from Jan 1, 2027 (base salary and bonus target unchanged, no new equity awards). Spencer S. Stiles, 50, 27 years at Stryker, Group President Orthopaedics from 2019, President and COO since January 2026, becomes CEO and joins the board Jan 1, 2027; board expands from 10 to 11. Stiles: base salary $1,320,000, 150% bonus target, February 2027 long-term incentive target value $14.2 million (40% options, 60% PSUs). The 8-K says nothing about a search process. Second source: Orthopedics This Week listing Oct 6, 2026 (https://orthotw.com/). Stryker appears in many prior drafts for products; no prior coverage of this leadership change (grep for Lobo, Stiles). Source
  • Neurosurgical Focus, pooled analysis of two randomized trials (211 patients): wound dehiscence 0% after biportal endoscopic vs 8.7% after microscopic single-level lumbar decompression — Park SM, Song KS, Kim HJ, Kang MS, You KH, Kim JS, Park HJ (Seoul National University Bundang Hospital, Chung-Ang, Konkuk, Hallym, Catholic University of Korea). Neurosurg Focus 2026 Oct 1;61(4):E19; doi 10.3171/2026.7.FOCUS26203; PMID 42822040. Verified against the abstract via NCBI E-utilities. Post hoc pooled analysis of 2 RCTs of biportal endoscopic (BESS, n=108) vs microscopic (MSS, n=103) single-level lumbar decompression or discectomy for stenosis or herniation. Primary outcome wound dehiscence requiring resuturing after suture removal: 9 patients (4.3%), all MSS (0.0% vs 8.7%; OR 0.046, 95% CI 0.003-0.798; p=0.001). Whole-cohort risk factors: incision length >=35 mm RR 15.44, drainage >=100 mL RR 8.55, WBC change >=2x10^3/uL RR 7.78. Within MSS, only drainage >=100 mL significant (RR 9.17, p=0.011). Authors call the findings hypothesis-generating and want validation in independent cohorts. Companion paper in the same issue (PMID 42822046, 58-patient RCT, IV tranexamic acid did not improve visualization in full-endoscopic lumbar discectomy) was considered and not used. No prior Morning Brief coverage of biportal endoscopy or wound dehiscence (grep of all drafts). Source
